North Carolina Law (NCGS 130A) requires children to be vaccinated for certain diseases before entering school. A certification of immunizations and a health assessment for any student new to North Carolina schools must be brought to the school within 30 calendar days of enrollment or the student is excluded from school until such time as they are presented. Physicians have access to required forms. Be sure to ask for both the North Carolina Health Assessment and a sports physical if your student will be involved in sports.
